New Employee Information
As you are probably aware, when you hire
employees you enter another realm of tax reporting and recordkeeping.
Nowhere are accurate records
more important than when it comes to your employee payroll records; not only for
the benefit of your employees but also for your own benefit.
PLEASE FILL OUT THE FOLLOWING FORMS:
FORM W-4 EMPLOYEE’S WITHHOLDING ALLOWANCE CERTIFICATE

You must give all new employees this form. Each employee must fill out
the form and return it to you. The W-4 shows their filing status and
how many withholding allowances they claim.
You keep the original W-4 and send a copy to this office.
FORM I - 9 EMPLOYMENT ELIGIBILITY VERIFICATION

This form is required by the Immigration and Naturalization Service.
An instruction sheet for FORM I-9 from the U.S. Department of Justice
has been included with the form. The completed form is to be retained
by you in your files along with copies of the required identification.
Form I-9 must be kept for three years after the date of hire or one year after the date employment ends.
FLORIDA NEW HIRE REPORTING FORM

State and Federal law requires employers to provide information on
newly hired or rehired employees within 20 days of their date of hire.
The form must be sent to the State of Florida. The address is on the form. Keep a copy of this form for your file.

Keep a complete record of how many hours each employee works for you on a weekly basis during the year and terms of compensation.
